ELIGIBILTY FOR EYE EXAMINATIONS AT HOME

Residents who are unable to attend a community opticians practice unaccompanied due to mental or physical disability are eligible for an eye examination at home.

If a resident is able to attend a high street practice unaided then they are not eligible for an eye examination at home.

Eligibility options for patients

Patients are entitled to a free NHS sight test if they are:

  • Aged 60 or over
  • Under 16, or under 19 and in full time education
  • Diagnosed with diabetes or glaucoma
  • Aged 40 or over, with a direct relative diagnosed with glaucoma
  • Considered by an ophthalmologist to be at risk of glaucoma
  • Registered blind or partially sighted
  • Named on a HC2 certificate for full help with health costs
  • Prescribed complex lenses under the NHS optical voucher scheme

Patients are entitled to help with the cost of glasses if they are:

  • Under 16, or under 19 and in full time education
  • Named on a HC2 certificate for full help with health costs
  • Prescribed complex lenses under the NHS optical voucher scheme

Eligibility options for patients and their partners

Patients are entitled to a free NHS sight test if they or their partner:

  • Are in receipt of Income Support or Pension Credit Guarantee Credit
  • Are in receipt of Income Based Jobseekers Allowance
  • Are in receipt of Working Tax Credit or Child Tax Credit and named on a valid NHS Tax Credit Exemption Certificate

Patients are entitled to help with the cost of glasses if they or their partner:

  • Are in receipt of Income Support or Pension Credit Guarantee Credit
  • Income Based Jobseekers Allowance
  • Tax Credit and named on a valid NHS Tax Credit Exemption Certificate
